REQUIREMENTS	SCORE	ADDITIONAL NOTES
College Transcript	2.5 GPA	Based on 12+ credits
AND
NACE Exam	76
AND
Copy of LPN License
Documentation of LPN Work Experience	750 Hours Min.
Two Work References from an Employer (1 from Supervisor)
AND
Prior to enrollment into the program-specific coursework, students must meet the following:
Pre-req courses	GPA of 2.0 or higher	Microbiology, Chemistry, Physiology, Speech and Social Science
